Please consider the implications to your project/initiative if New Zealand is required to operate under COVID-19 Alert Level 3 or 4.

Account Details

If you are successful you will need to provide a screenshot or scanned copy of your bank statement. This will need to include the bank name or logo, the account name and the account number.

  • You are authorised to act on behalf of your organization (if applicable)
  • The information provided in this application form is true and correct to the best of my knowledge
  • That you will fulfill the requirements as outlined in this form to the best of your abilities
  • You understand and agree to complete and submit a report about your project/initiative within 10 weeks after the completion date. A reporting template will be provided should your application be successful
  • If successful, you will need to supply the Ministry for Pacific Peoples with a scanned copy or screenshot of the bank account number that the funds will be deposited into
